Where do you find the card rebate besides the Easy saver one?

on the front of the package in bright yellow and red it says SAVE $3.00 mail-in rebate offer on back-the form is on the back of the package

I didn't see any mail in rebates on the back of any of the packages I saw in my WG store.

Did anyone ever get this rebate? I'm pretty good about tossing my copies when I get the check, and I still have this one in my folder. My records show I sent it in on 1-10, so it's been a long time! TIA
