To obtain your $1.50 refund, please send the:
1. Entire bottom panel of outer carton, including
UPC symbol and product name.
2. Cash register receipt from the purchase of
Orajel with the price you paid circled.

Same here. I just peeled the label up and cut out the UPC and a part of the bottom that mentioned Scooby-Doo. Hope it works!
