Proof of purchase must accompany form...states....RECEIPT(S)
Must have purchased between 11/1/2010 and 12/31/2010
NBPN (no beer purchase nec.)just mail form and receipt(s) in

Thats pretty much it. SO if you bought a gift card for 25.00 or more
mailing this form will get you 5.00 check back. Requests have to be postmarked no later than the 15th of Jan. So hurry and send your address
