do you have the customer service number so I could call to see if they consider formula a non-food or a food item. Sometimes I've been surprised (you would think that it is ALWAYS a food...um...I guess some places don't see it that way...who knew?)

Hi! There isn't a number listed on the rebate form to call. However, the rebate does state "for questions or to check on the status of your offer, please visit cmspromocheck.com". When I went to that website it stated "You can contact our consumer care line at 877.322.8355, Monday – Friday, 8:30 AM to 4:30 PM EST". Hope that helps!
