Law prohibits use by persons enrolled in or for any prescriptions reimbursed by any government (state or federal) funded health care programs, including Medicaid and Medicare. Not valid for any prescription previously filled or transferred from another Cub Pharmacy location. Not valid if a copy, facsimile or reproduction and non-transferable. Cub gift car not valid for use on prescription purchase. Limit one gift care per customer.
