I've confirmed that you do NOT need the form, according to "MaryAnn" @ the Strongsville office listed in this thread. You will have to call them if you do not have/get the form (cat machine is down, cashier forgets to give you form or store is out of forms, etc.) when you purchase the items.

*This year, you have to purchase the items in the designated time period by the store.
    Example

Kmart and Marc's are participating and you get the form via a tearpad.

Marc's purchases time period is 9/25-10/5.
Kmart's purchase time period is 9/30-10/31.
Giant Eagle's purchase time period is 9/30-10/6.

Seems a little tricky this year....how is it that there's specific purchase periods but the form says the promo runs much longer??
