Which grocery store was this for? Is it possible just to get the form online? I didn't see anything about the bill payment when I googled tombstone pizza.

It is a promotion from Tombstone. It is not for a grocery store; Tombstone pays a utility bill for you.

Try Googling Tombstone Rebate. I found it online, but it is a pdf and I don't think we are suppose to post those here. It comes from the Kraft Foods website, but I couldn't find
