1. Collect Box Tops from Country Hearth items bearing codes CH01-CH08 found on each coupon.
2. Attach them to the front of the form*.
3. When you fill up all five spaces, you will triple all Box Tops values for a total Bonus Card value of $1.50 !
4. Turn in your completed Bonus Card to your school box Tops Coordinator.
5. Start all over again


No photocopies or other reproductions will be accepted. Bonus certificates may not besold, exchanged or traded and can only be redeemed by an accredited K-8 school registered in the Box Tops for Education program. This offer expires 12/31/09
