Yes, the sheets were mailed to your school BTFE coordinator. I am a BTFE coordinator for our small school and was mailed a number of these for my school.
I saw this on the BTFE forums. There is a new collection sheet out now that is for 30 box tops and when you send it in they will automatically DOUBLE it!
Limit of 2 per household.
Must be mailed by October 31.

I just sent in 60 BTFE we collected over the summer. I have to go to the PTO meeting this coming week and urge her to use these sheets for September.

Very cool! I printed out two to fill out for my kids, and one extra to send in blank in case the coordinator hasn't gotten one, to make copies to send out.

I'm our school coordinator and I didn't get any information about these. I tried following the link and I can't log in, so I can't print them out.
This is NOT the same log in info I use to enter the coordinator site.
If anyone can help me get these forms printed, my school would greatly appreciate it!
If you look at the discussion thread posted in the OP, I think about halfway down the first page someone has posted a link to a printable page. Instead of clicking on the link (which takes you to a login) - copy and paste it. Then find the 'collection sheets' link under the BTFE section.
Thank you for posting this!! I'm going to be getting together the October submission since our coordinator is on maternity leave. This is my very first time doing this, so every little bit helps! How many sheets can I send in? Would I be able to use one for every 30 boxtops?

These were mailed out to the school box top cordinator.It was great the school got extra BTFE .But I did not like that you had to put your name & address on them.I just put the box tops that I had collected on them and had the teacher put there address on them.We only got 30 forms in our package.Not sure how many sheets you can send in.It did not say there was a limit in the package.
As far as I understand it, and it states on the sheet that you can send in up to $20,000 per school per school year. You can send as many as you want but ONLY 2 SHEETS PER HOUSEHOLD.
I sent in 4 sheets with my daughter, but 2 have my name & address and 2 have my mother's name & address. Since my mother definitely contributed at least 1/2 of these boxtops there is no fraud to this. If your students have their relatives help collect boxtops I don't see why the couldn't use their household information on 2 forms. Also, the teachers & staff at the school collect boxtops for the school and each of them could put their info on 2 sheets.
