Don't be too annoyed! Others have posted that they have called about expired rebate forms, and were told to send them in even if they were expired and they would honor them. HTH

I have mailed off several and blacked out the coupon and the total paid. I also blacked out my card number too. I haven't gotten anything back because I sent them about 2 1/2 weeks ago. I never thought to leave the end user info blank for whoever I donate them to. Great idea!!!

I'm one that called because my rebate form was expired. They said just go ahead and send it. So I did, and am waiting for my first check to arrive!

You send them in, but you DO NOT fill in any of the information. This way they know that this particular monitor is being donated and will potentially have someone calling in with the corresponding serial number in the coming weeks/months.

My box on the Breeze monitors has the sticker on it, but all I can find inside the box is the registration card. Where should I be looking for a rebate form. The boxes were all sealed.

The Breeze ones were INSIDE the plastic sealed with the instruction booklet in mine. I just did them a few minutes ago, so I am sure!

Oh, and in the Contour boxes, they were loose inside the box, not sealed in with the booklet... HTH

I got one of my rebates back today! just waiting on the others and I have another 3-4 to send out
