Gerber Graduates $5 offer form...need help please

The Gerber® Graduates® $5 Coupon Booklet Offer requires one form with each 20 UPCs and must be sent in before the expiration date.

Unfortunately, at this time, the offer forms are not available upon request

Alright, my oldest is now 5 so it's been a long time since I've done the graduates offer. I recieved a form (mail, hospital, IDK) it states you have to redeem 20 UPC's for the offer....last time I did this offer it was 10.

Since it seems the program has changed a bit I have a couple questions.

1st....Can you still submit multiple requests per form? Is there a limit? (other words, can I send in 40 UPC's and get 2 books?)

2nd...Where can I find more forms if I need them...there was a PDF years ago, but I don't see one now so is my only option to trade for them?

3rd....The forms we used when oldest DS was a tot were taken even if they were expired, is this still the case with the new forms?
