Need help on Heineken rebate husa 8276

This is the one where you get $20 wyb beer and $100 worth of groceries. Looks like it is good in most states including DE. I ordered a couple of these and did not realize there is a line for the retailer where the forms were found. If someone could plmk a retailer name/city/state where they found these, I'd appreciate it soooooo much. TIA! Mel
