Dos Equis 25/50 Meat Rejection?

So I think I was rejected, but did not receive any info as to why. I received my form & receipt back in the mail today, with no explanation. Here are the two weird things:
1-The envelope came from scout.com in Seattle, WA, not the MN address where the rebate was sent (and scout.com is a sports website)
2-It was hand-addressed to my maiden name! The form clearly has my married name (which I changed 1.5 years ago) I have submitted rebates at some time in my maiden name, so maybe they use a db...but still weird.

I can't find any Dos Equis rebate contact info. I am in a NBPR state. The postmark date is 9/17, so I have some time to re-send, but not sure what the heck is even going on or what I did wrong. I just mailed it on 9/3.

Anyone have any ideas on what I can do or have something similar happen?

Sorry, I have no ideas on what to do, other than dig up the Dos Equis rebate contact info... is there any info on their website? I have to say, that is very odd, that it would be hand-addressed to your maiden name, even though you used your married name, and that the envelope came from a sports website in a different state than where you mailed it. Could you have mis-mailed it?

Do you think it got lost in the mail and someone put it in a new envelope and mailed it to the only address they could find?

I just came back to read this today. I thought I had this board setup to email me if anyone replied, so I thought I was initially ignored! Anyhow, I ended up emailing Dos Equis and they initially responded that they'd be happy to look into it for me, they just needed my mailing address. I immediately sent it back to them. Then I heard nothing. I re-replied to the email every other week or so. Today I resent another email. So I still have no resolution. I may have to look for a phone number. I am not giving up on $25 that easily!!

So, what happened? Did you ever get your $25 from Dos Equis? :)
