Received reply from Heineken today:

To do so, please reply back with the following information: Address Rebate/offer details Dollar amount of your rebate Date rebate was mailed Upon receipt of this information we will research your request. Used the contact us form, the e-mail the reply comes from is heineken@qualitycutomercare.com

I'm unable to check my rebates here, I add my info click and nothing happens.
Just want to know if anyone else is having this problem or if it's a problem with my computer.
