    Default Sure Max Rebate form question

    Hi Everyone.
    Does anyone have the original Sure Max rebate form that does NOT have the TRY ME FREE shown on it? I don't know what I did with my form. I bought the Sure Max that does not have the TRY ME FREE label on it so it doesn't qualify for the new form. As near as I can tell, this form was available in September 'cuz that's when I downloaded it.
    Please let me know if anyone has it.

    FYI,
    If you use the old form you may get a letter back from the company saying they won't send your rebate b/c you didn't attach the Try Me Free label.
    I had the old form and sent in for the rebate and got that letter. I'm going to fight it since I have photocopies of my original mail-in stuff. Just wanted to warn you that it may not be worth the headache!

    BTW, can anyone tell me who I should threaten to file a complaint with if my rebate is not issued to me? I'm not going to put up with this bait & switch, changing rebate form wording in the middle of the offer...uh, uh...
