looks like this needs to be 10 separate envelopes.

For each $5.00 rebate by mail, mail this form along with UPCs cut from
each box and the original store register receipt(s) dated between 3/17/08–4/30/08

to the address provided above

No separate envelopes are needed when doing more than 1 of this rebate. The form has a blank to be filled in (next to zip code) "# of rebates requested" and other language that makes this clear. But again, this sort of guessing rarely is needed in rebating; just call the manufacturer if you have a question; almost all manufacturers have toll-free numbers that can be obtained via 1-800-555-1212 or via on-line databases.
