How irritating! I just spent 40 minutes on hold waiting for a customer service rep at that 800 number in the hopes that they would send me out a rebate form. Nope! They just said it should be with the bottle. When I bought mine, there was nothing on the shelf and no peelie form on the bottle.

These were gone long ago at my local CVS...the only place that I have seen them in my area. I guess I will try with the 1-800 number

I still have not heard back about my MIR. Its been about 4 weeks. Should I be concerned?

I wouldn't be yet, I just checked on the forms, and it says 6-8 weeks. Give it at least 8 weeks before contacting them.

I found these at CVS last week - also got the Smart Rinse free after ECB! YAY!!
