There is no "S" on cash register receipt. The form says "See your Walgreens pharmacist for more info on these smoking cessation products", but does not say the products have to be purchased at Walgreens.

Is there a phone # listed? I was wondering if I could call them and ask if it had to be purchased there (just to make sure) and also if I am unable to locate the form if I will still get the MIR offer if I just write it out. Thanks!

6 months later - here's your answer! LOL - I accidentally stumbled on this rebate post, and emailed them. Here is the response:

We can accept your refund request without the form. You can submit a 3x5 card with your full name and mailing address along with the upc off the package and your cash register receipt to the address you have included in your email.
