I found it on a beer display in the deli. You do need the form to do the rebate. You also could check the trading forum and see if anyone has it available for trade.

Hi, I have a quick question about this rebate.
This particular rebate form reads:
off the purchase of any ONE (1) Hormel Party Tray OR any DELI ITEMS
(minimum 15.00 deli purchase).
Does this wording mean that I could just buy any deli meats or cheeses, not Hormel, just any, and still be eligible for this rebate? Or does it have to be Hormel meats? So confused
Am I reading this correctly? If anyone knows the answer to this, will you let me know? Thanks so much!!!
