Form is available as a .pdf you can save to print when you have the receipt and UPC. There is also one for $15 here which requires purchase of 56 oz bag of M&Ms and a 24 pack of the Orville Redenbacher's popcorn. Please note, both forms indicate the M&Ms must be "specially marked" so it seems you can't just buy any old bag of M&Ms.

so are there specific bags of m&m's with "entertainment cash" on the package I have looked all over locally and don't see anything(?)

Yes. The packaging does say "Entertainment Cash" on them. I have only found them at a few stores so far and they have been on the larger 19 oz bags. I have yet to see them on the 9 oz bags.
