The form is a small peelie attached to the Nexcare boxes. I found it at Walgreens this week, on all three included types. It's maybe an inch square the way it's folded up, and yellow if I remember correctly.

I finally received a Nexcare rebate- but the amount was for $6.19. I am not sure where they got this total from? I bought 3 different kinds, and they were the Walgreens sale price each ($2.49? $3.49? can't remember). The prices don't add up

Six weeks today since I mailed mine off. Nothing shown up yet, but perhaps it took a while getting to them in them in the mail. That was the same week two items sent to me went astray. Took a full two weeks for them to arrive here. These things happen in threes, am I right?
