Just a little intrigued because the form allows up to 8 different UPCs. They are assuming the average unit price is $3.75 or more. Guess buying those $2.50 Pantene clearance items at CVS is out, unless combined with some more expensive items to bump up the unit average.

I wasn't using the cvs ones, but had to add to mine to get to $30, I just wrote in the extra one at the bottom.

Save a copy on your computer (I always save stuff like this - I have a folder just for MIRs and store policies and such). Open the saved file, then in the menu go to Tools, then Select & Zoom, and select Snapshot Tool. Highlight just the form part (you might get a popup that says "the selected area has been copied", just hit OK), then hit Print. The preview will show just the part you copied. I printed it just a few minutes ago and it came out fine.

What I did was after I opened the form, on the left side, in the middle, looks like 2 sheets of paper, click that. 2 boxes show up, Layer 1 & Layer 2. Click the eye in the box for Layer 2, and it makes the top half disappear, so only the form will print. Doesn't do anything for that purple blob on the form part, but I printed in b&w draft, so I didn't use any color for that.
