To receive $5.00 rebate in those states without beer purchase, you must purchase at least $5.01 worth of Terra Chips ,send this form and the cutout of each upc from the qualifying products along with a CRT. Indiana residents are eligible for a $5.00 rebate with the purchase of at least $5.01 worth of products that are non-alcoholic beverages if the retailer does not carry Terra Chips.

PA & OH residents: discounts will be applied to the beer. Louisiana residents are eligible for a $2 rebate only. So sorry for the delayed response! This is a form that you find at the grocery store. Sort of like a tearpad. Usually found in the beer section. Yoou have to have the form to receive the rebate (copies are not usually accepted)!
