There are multiples of this one out there! Mine is SI - Northeast; DGUSA0039. Same states as noted above.

Yes - I just learned this today, there are multiple versions of this form.
I confirm mine (original post) are DGUSA0037 and I am sorry I didn't set up the thread title and DB properly with the NBPR states....

i have a question for everyone....do you think it would be ok if i submit a receipt with 2 pizza's equaling $5 (they are on sale at giant eagle this week) or do you think 1 pizza has to be $5? i've never done a smirnoff rebate before....

I can't get to the form at the moment, but I think your pizza or pizzas have to be at least $5.01...
