A few days back I found a tear pad by the Coke 2 liters. it is called OFFICIAL 2009 HALLOWEEN FAMILY DOLLAR $10 REBATE FORM

To receive rebate, send in these items:

Completed form (no photocopies)
Original UPC codes from 5 - 2 liters of Coca Cola products
Original UPC code from 1 Halloween Costume (retailing $10 or more)
and Original Family Dollar receipt with these purchases circled.

Rebate requests must be received by 11/30/09 - purchsed between 10-01-09 and 10-31-09.
