To receive your $3 rebate by mail:

• Purchase one box of Minute Rice and one of the participating Wubbzy DVD’s by September 30, 2009.
• Mail this completed form, the original store identified cash register receipt dated on or before 9/30/09, and the UPC symbols from both the DVD and the Minute Rice.
• Your request must be received by October 30, 2009.
