    CHECK x7/31/09 PediaSure

    Found in Fla. paper:

    MIR: PediaSure 6pk

    - Buy 1 6pk of PediaSure
    - Send in MIR form w/ nane, address and zip clearly printed
    - Include original dated register receipt w/ purch price circled and origimal UPC symbol fron PediaSure protective over wrap.
    - Form needs child's b-date.

    There is also a rebate form online. I printed it from the Abbott/Pediasure website a couple days ago but now can't find it on the website. If anyone finds it, please post the link.
