I got my form returned today as well and it took a month to get back.
It's too bad I traded for three of these and now I can't use them. I will call tomorrow and see if I can confirm the end date so we can get the database changed or at least linked to this post so that other people don't trade for them as well.
**update: yes, the dates were printed wrong on some of the rebates. The PO box has been closed, but they will still send you the $5 check if you call the number on the rebate and read off the wrong dates.
