$5 Mike's Hard Lemonade: To redeem offer purchase must be made in a single transaction. The first rebate is valued at $3.00 on the purchase of meat (excluding deli meat, min. $3.25 meat purchase required). The second rebate is valued at $2.00 on the purchase of cooking utensils (minimum $2.25 cooking utensils purchase required). No Mike's Hard Lemonade Co. purchase required. Complete mail in rebate certificate and send, by regular mail: 1) completed mail-in rebate certificate; and 2) original dated cash register receipt, with prices of meat product(s) and/or cooking utensil product(s) purchased to: Mikes Hard Lemonade Co. Rebate, CMS Rebate Services, MHL63, PO Box 426008, Del Rio, TX 78842-6008. Offer valid only on purchase made between 5/1/09 and 05/31/09. All claims must be postmarked by 06/15/09 and received by 06/22/09. Valid in AL, IN, KY, NC, MO, UT, and WV.

This is one rebate form with 3 check boxes. The first check box states you enclosed original package UPC code(s) and original eligible cash register receipt showing the participating meat product(s) and/or cooking utensil product(s) purchased. Second check box is to request the $3 meat rebate. Third check box is to request the $2 utensil rebate. So you would check both boxes to get the total $5 rebate.

Any one else still have this outstanding? I mailed Jun 1 and have never received it. Of course this one had no phone number.

I just checked on the website and it says it was received 7/25 but is still in process.
