There are two different versions of this form.
There's also one valid in AL,CT,MA,ME,MN,MO,NC,NJ,NY,RI,SD,UT,and WV. It goes to PO Box 750057.
ETA: This rebate is good on Charcoal, Ice, Grilling Utensils (lighters) and paperware.
