1. Mail this original certificate (no reproductions) with your complete name and address handwritten on a separate 3x5 card
2. Include the 12 digit UPC bar code cut from the carton
3. Include your original Cash register receipt dated between 9/1/08 and 6/30/09 with the poduct and purchase price circled
4. refund will be up to $4.99
5. Envelope must be postmarked by 7/15/09
