x11/20/2008 AUTOZONE GUMOUT REGANE REBATE – Limit two, 10/22/2008 -> 11/19/2008
$6.99 each x 2 = $15.98 rebate.
They have this item as a FAR frequently, lik five times a year.
Stated on the front page of their 10/23/2008 -> 11/19/2008 store flyer (lower right hand corner of the page) …

Not valid in CT, RI or Puerto Rico
(Both CT and RI have new rebate laws that if it is a FAR, that the customer must received the item for free at the checkout and the store must submit the rebate on their own.

Some changes that I have noticed …
• They are asking you to check off a box that you understand the terms and conditions.
• They ask for your autograph (okay, your signature)
• Looks like a new mail-to address:

I emailed AutoZone asking if this offer was limit one or two, since:
• The rebate form sales limit one
• The large window poster at the store says limit one
• The store flyer says limit two

Their reply says TWO:

You can send the rebate form in for the purchase of two Gumout Regane Complete Fuel System Cleaners.
Just be sure to fill out the form for two bottles of Gumout Regane and circle the purchase of two on your receipt.

Well, the form only allows for one not two (the previous versions of this rebate form had a section where you did the math, price x quantity = total rebate amount [$6.99 x 2 = $15.98]), so I am wondering if I need two separate forms.
Whereas, their email reply uses the word 'form' in the singular.
