Does anyone have any idea if the item has to be purchased from the "deli". I was thinking of buying some cheese and a rotissery chicken from Sams Club. Do you think this would qualify?

Tracey - those def qualify - I get the rotisserie chicken and a deli meat all the time with these rebates- no issues!

Do you have to have the actual rebate form to do this? If you have done this without the form did you receive your check yet? Has anyone ever had a problem with not getting their refund back? Tia..

this might be a silly question, but has anyone ever tried submitting a receipt from say pizza ordered out (like Papa Gino's or Pizza Hut)? And if so, have you received the rebate?

I'm afraid it's not going to work for this mir. It requires either take-n-bake or frozen pizza purchase. I did a few mir's which says ..., pizza,... with pizza hut reciepts and always got my checks without problems.

I would say only pizza from your grocery store would count on this one...
