I found it as a tearpad in a local store called Bloom ( sister store of Food Lion)

I am not aware if there is a online version.

Judging from the UPC codes, the new organic Good Earth coffee is made by Eight O' Clock coffee. I'm wondering if they will start to include Good Earth in their Accumul8 Rewards program. I tried it today and it didn't work, but, they may include it later. Doesn't hurt to try.

I found the 20+ I have at a Winn Dixie grocery store, one that I normally don't visit in fact.

Can someone tell me what info is required on the form? I haven't been able to find one but I'll give it a shot without the form and see what happens.

UPDATE:
I emailed the company to find out if I could submit the request without the form, and they emailed me back very quickly, and said that if I can't find another copy of the form at the store, they could just send me a coupon for a free bag. That works out great for me, since the rebate was $5 and a bag sells for $6.99 in my area. I used the Contact Us link from their website.
