The box has the same rebate form, that is where I got mine. They are on sale b1g1f at publix until today ot tomorrow (depending on your area). Anyway, a little OT, but these are great! I do not like FMW at all. I opened the package to get the rebate form from the inside of the cardboard and they smelled so good I couldn't resist. I ended up eating probably 1/4 of the bag, then went back and got 8 more at b1g1 with the $1. q's someone mentioned below from all you.

That link has spaces in it (in the word start and before the M in Mail) that is why it doesn't work. Try this: http://www.smartstart.com/swf/Mail-In_Rebate_Offer.pdf
