Was not able to get the actual form for the Blueberry Frosted Mini Wheats, but did get the one for the Smart Start. I did not much care for the taste of the Blueberry...was too over powering...so am mixing it with regular frosted mini wheats so as not to waste it. Was surprised since I love Blueberry.

Does anyone have an extra TMF form printed out? They don't come in the boxes anymore and the link to print out online is dead. I've tried contacting Kellogg's directly via email and they won't respond. Why make a promotion good until july 2009, when you won't give ppl the forms to use! UGH!
