Did anyone else notice that the TMF rebate form for Zout doesn't have an address on it?? Any ideas where to send it to?

I sent an email through the website's contact us function to ask. I'll edit this if they get back to me.

Update: Never mind. I just printed the form again on another printer and it was fine. Address and expiration date showed up this time. I think maybe my printer needs more ink!

Did you notice that it says they require a copy of your CRT? It does not say original so if you buy 2 MIR items at WAGS with the $5/$20 coupon you can send them both in without having to worry about rebate receipts.
