The MIR form shows the yellow/blue striped Spring Fling card on the front, it was on a special display near the gift cards. Keep your eyes open for this AND GET TWO FORMS if you are going to submit this twice as there is only room to put the info for two gift card PER MIR form. You will need two forms to submit for four gift card if you want 2 Spring Fling cards!!

Thanks! Did this last year when they had the forms available at Safeway and CVS. However, last time the GC that we received was good at Linens & Things, Old Navy and one other store (can't remember now)......not the ones mentioned this time.

Wow... bummer on the restrictions on where to buy the cards. My bank waives the activation fee for those over 50 (and dh is!)... still a good deal with gift season coming.
