The Advantage Buy insert from today's paper had a blurb about the $5 GC with purchase of 5 Kelloggs/Keebler/Sunshine items and said the form would be available in stores and ONLINE!!!

The one that was in the newspaper in other areas (10 items, $10 rebate) definitely said 1 per household; so, I had just assumed this one did too. Having read it more thoroughly, it certainly does not say that but they give themselves lots of wiggle room with that "we have the right to limit" blah blah blah
I dunno- looks pretty YMMV. I probably won't "risk" it- not worth the effort and stamp for me
So we just have to circle the items? No Proof of Purchase required? Yays!
