Free Go Phone?

A couple weeks ago I came across a tearpad with forms for a free go phone from Firefly wyb certain items from Publix.

I have misplaced my form - !!!! - and can not remember the shop by dates and such. I haven't found it here in the forum either.

Does anyone know what I'm talking about?
