In my county (pretty sure it is state wide though) tax is PRE manufacturer Q, but POST store Q) so you would pay tax on $4.99 (better than $10.99, but not as great as 1.99) We don't pay tax on meds. Only non-food or prepared foods. Food and meds are tax-free. Out tax rate is 8.75% for non-food stuff, 4.75% for clothing/diapers.
Hope that is a little helpful. Remember that these stores that have this triple Q, get no other doubles normally.

If I buy a taxable item that has a regular price of $2.99, is on sale for $1.99 and I have a $.50 Q that the store doubles, then I pay sales tax on $1.49. The rationale behind this is, first you only pay sales tax on a sale price in any store in VT. Second, since the store is paying the double part of the coupon, my state considers that an even lower sale price given by the store on the item, so I don't have to pay sales tax on that part. Third, any Q I have seen states customer must pay all taxes, etc. so I do have to pay tax on the amount prior to the Q being applied. It's a little backward but that's the way it should work.
