No, I don't think so. The form doesn't say anything about buying at a specific store. I just happened to find the form at petsmart. I thought I saw the same form as a printable online but cannot find it again, so maybe I'm just seeing things. I haven't been to Petco because their prices are always atleast $1 higher than Petsmart but since there is one close next time I'm next to it I will check there too.
