Office Max: Question on maxperk rewards

I am such a procrastinator about rebates...I finally registered online from the awesome printer deal last month and it says that my 30$ rewards has been issued, but i just today put in my address, so will i get an actual card for the rewards or can i use the print out statement for my purchases? It has a card number and pin number listed on the the card, so I am thinking i can just bring the printout in to the store? I didn't see any info on this in FAQ's online
