    Default COMMISSARY the smartspot quaker rebate

    Is there somewhere to download the form or print it off or do I need the tearpad? I sent dh to the commissary but he can't find it (not sure if it's not there or he can't find it ) and I never go because it's too far from here.

    Thanks for any info, I know what to buy and when it ends but don't have a form.

    I haven't seen it at my commissary (nor any of these rebates from Quaker that Commissary people talk about!). Someone sent me one as an AOK.

    1. Buy 10 Quaker, Tropicana, Dole, Gatorade or Propel branded Smart Spot products excluding Quaker Instant Oatmeal Express.

    2. Send: a) This completed Official Mail in Offer Form, b) original commssary-identified, itemized cash register receipt(s) dated 3/1/09-4/30/09 with Smart Spot product purchases circled, and c) the original UPC code from each participating product (For UPCs that cannot be easily removed, write the corresponding UPC code on the original cash register receipt) to:

    Smart Spot Rebate Offer
    c/o Harthill Marketing Services
    4403 Meramac Bottom Road, Suite A
    St. Louis, MO 63129

    3. Receive: $5 check by mail


    Requests must be received by 5/15/09. Allow 8 to 10 weeks for shipment. Offer valid to customers of U.S. Military Commissaries and Exchanges only. Theft, diversion, reproduction, transfer, sale or purchase of proofs-of-purchases is prohibited and constitutes fraud. Void where prohibitied, taxed, or restricted. Requests from P.O. Boxes or clubs or organizations will not be honored and proofs-of-purchase will not be returned. The Quaker Oats company reserves the right to verify identification. The Quaker Oats Company not responsible for postage due, lost, late, damaged, misdirected, illegible, incomplete or undelivered mail requests, or any failure, unavailability, interruption or other problem with any electronic communication. Requests must be forwarded in an envelope with sufficient first-class postage. Fradulent submissions could result in federal prosecution under the U.S. Mail Fraud Statutes (18 U.S. Code 1341 and 1342).     I wonder too...I just got 20 coupons off ebay. I will break it up if I can do it twice.

    No I can't find it and the terms listed here are vague. They don't say no per more than one but do say they have the right to verify identification. Unless they mean make sure you have military id? I'm not sure. Thank you! I will be doing it twice! woohoo mm.
