COMMISSARY the smartspot quaker rebate

Is there somewhere to download the form or print it off or do I need the tearpad? I sent dh to the commissary but he can't find it (not sure if it's not there or he can't find it ) and I never go because it's too far from here.

Thanks for any info, I know what to buy and when it ends but don't have a form.

I haven't seen it at my commissary (nor any of these rebates from Quaker that Commissary people talk about!). Someone sent me one as an AOK.

1. Buy 10 Quaker, Tropicana, Dole, Gatorade or Propel branded Smart Spot products excluding Quaker Instant Oatmeal Express.

2. No I can't find it and the terms listed here are vague. They don't say no per more than one but do say they have the right to verify identification. Unless they mean make sure you have military id? I'm not sure. Thank you! I will be doing it twice! woohoo mm.
