You input your telephone number and then are connected to a live operator (by a subcontracted company) that will ask you feedback questions (on a scale of 01 to 10) that will last for 10 to 12 minutes.
At the end of the interview they will ask for your NAZT (Name, Address, Zipcode, Telephone Number) so that they can mail you the $5.00 KMart gift card.
