I found these forms at Penney's today.
Receive a JCP GC by mail valued at $5 or $10 after your purchase of 3 or 5 Hanes products.
1. Complete mail in certificate
2. Enclose JCP receipt with 3 or 5 Hanes items circled
3. Enclose UPC from each item
4. Mail to Hanes/JCP Offer, PO Box 8213 Dept 21, St Cloud, MN 56398-8213.

Purchases must be made by 8/2/09 - 9/5/09 and PM by 10/15/09. Limit 2 redemptions per household.

There is nothing on here saying it must be a certain dollar amount and nothing states that it can't be used on clearance items.

Where do you find the forms? And do I have to get one from the store, or can I find it online. I am just thinking that if I go up to the Customer service counter or my cashier they'll look at me like I'm crazy and have no idea what form I am talking about.

I haven't checked online. But I found the forms right above an endcap of girls underwear.
