If what the PP said is true, I'd prolly go to the BBB. They may not be able to get resolution for you, but the more marks against their name, the less mfr's will want to deal with them and the less likely they are to continue this baloney, kwim?
It doesn't help to fill out the form & submit it because they don't ever respond! I wrote them 3-4 times and no response.A direct email address is contact@amcglobal.com I also left 2 phone messages. I was supposed to get 3 of those pre paid cards for filling out surveys for Kraft Cheese, sugar and Bull's Eye BBQ sauce. I submitted my surveys mid June. Prepaid cards were supposed to arrive within 4-6 weeks. Here it is 3 months later, and nothing.
I called the Corporate office today : 610.238.9200 x 3207
and talked to a live person. I asked her what the status was on my cards and she went to check (supposedly). She said they had already emailed me a response and to check my email. I told her no email from them had ever come (I am certain of this because I set up a separate email account for AMC surveys). She told me to email again which I did. I also faxed them- fax: 610-238-4460. No response.
Here's is their website:Attitude Measurement Corporation
I agree with queenofthehivemomof5. They're somewhat of a scam company. Normal companies would at least give a reason for denial.

The owner of AMC Surveys is Rob Roshkoff (that's what the girl on the phone told me & I verified it on the website).
His email is riroshkoff@amcglobal.com
His phone: 610 238 9200
his fax: 610 238 4515.
Hope the helps, mommy22boys! You might try emailing Mr. Roshkoff directly. I know they won't answer mine but maybe they will you.
I received one of those cards from a Biore survey. It also had the Maestro symbol. Before I realized that it wasn't just a regular MC or Visa, I tried to use it at Staples. It was declined, but I didn't lose any funds. I did some reading online and found that others with the same survey card were using theirs at QT for gas. It did end up working for $15 worth of gas. I'll probably not participate in something like this again because of the restrictions, limitations etc.
One in particular that was troublesome was that the instructions stated it was a one time use... use the whole $15 in one transaction. If I didn't, I'd lose what was left. Big PITA.

Also, QT pumps do show the Maestro symbol. So I'm thinking that even though Maestro is MC, it can't be used unless the Maestro symbol is accepted by the retailer.

Thanks for all the advice. I called the # to check the balance just in case like PP said, it was declined because it's Maestro, not Mastercard, and Hyvee doesn't display that symbol. Well it said my balance was $12. WTH? It also asked if I wanted to remove the funds from my card and direct deposit them into my bank account. Really weird since the paperwork I got never mentioned that. It went on to ask for my bank account info. Ummm... no way in hell would I give that info to this company! I don't even think this number was the same company as the other number!
I didn't realize til I read this post, then dug up the letter they sent with the card that the whole $15 had to be used at once!! I freaked a bit b/c I used it at Winn Dixie for a $5.25 purchase. Just checked my balance, thinking it might be zero, but is $9.00. So I lost .75.
Tomorrow I'm going to Target, so if its accepted there, I'm getting a $10 GC. Pay $1 in cash and use the rest towards the GC.

When I called I talked to a girl and said these cards are difficult to use. She also said that each time you swipe the card and it is denied the card gets charged a fee. She refunded my fee on my card since i tried to use it and it was denied. She explained to me you have to swipe the card, select debit, enter your pin number and when it asks if you want cash back dont select no. at this point i am to tell the cashier i only want to pay $15 on that card and the cashier is supposed to enter the $15 amount. She said do not tell the cashier in the beginning that you are only paying $15 with the card. She said if the cashier starts pushing buttons before you swipe the card it will throw the card out. its confusing to me. I dont know why these are so hard to use. She did say that if i wanted she could mail me a $15 check. I told her i would try to use it one more time with the instructions she gave me. if that doesnt work i will call her back. I will go to walmart and try to use it today. i plan on using it to buy another gift card. i'll report back after i try it.
Ok I went to Walmart and i bought a $15 gift card with my maestro card. I just told the cashier i wanted a $15 gift card. I then swiped my card and entered my pin. Then I had to select no for cash back. It went through!I did not tell the cashier i was using a prepaid card. so she didnt enter anything in the register other than the $15 amount of the walmart gift card i was buying.
