I think they've pulled the MIR form from the site. I contacted them asking about a MIR form. We'll see if I have any luck.

The MIR form is still there.Click on the link in the OP's post and almost at the bottom it says "The best you've ever used guaranteed or your money back" click on that and another page will open with the MIR form.:)
