I got denied on this one also. I re-sent copies of all my documentation and was sent a 2nd denial letter. I finally called customer service for Church & Dwight, the company that makes Oxiclean. The lady at CS was very nice and explained that the rebate was not for the product that the "try me free" sticker was attached to. The rebate was for the mini spray-away bottle, which you had to buy separately. Very deceiving practice if you ask me!! They did end up sending me a check. So, if you are getting denied, it might help to customer service.

It's not store specific. The forms are little stickers on some of the trigger bottles.
