Is there a form for this? If so, where? I have a really nice co-worker that keeps a jar of candies on his desk for us ladies to partake of and he's got to be running out of money! I keep giving him coupons tho :)

Where did you find this rebate? Have a candy receipt from CVS I could use on this one.

There is a form. I found mine at the Kroger near my house hooked around the wine bottles.
